Add a Theme for your Survey in the form of Headers, Footers and Style Sheets.

Please Note: You must have the appropriate permissions to manage and view Surveys in the relevant ticket area, these are to 'Send Survey' and 'Complete Survey'

Guidance

Navigate to the Admin Area > System Areas > Select the system area e.g. Ticket > Surveys > Themes

Give your Survey Theme a name
